data; however, because of challenges related to the volume of information and limited ability to access archived files during the COVID-19 pandemic, PG&E requires additional time to
collect and review the necessary data. Ongoing:investigation into the cause of the bolt failure at the top of the butterfly valve seat ring and is preparing an assessment report to
document field findings, material testing results for the failed bolt, and an engineering evaluation of the remaining seat ring bolts. PG&E also completed its annual full-travel exercise
of the valve in August 2020. Leakage and monitoring observations during the exercise will also be noted in the assessment report. Because of delays in scheduling the investigation,
additional time is needed to complete the report and provide the results of the assessment to the Part 12D IC for review and comment
