Schedule COVID-19 Vaccine
Appointments Online
MyTurn is the state's COVID-19 vaccine scheduling tool
that individuals can use to schedule an appointment for
a COVID-19 vaccine or booster at a nearby clinic or
pharmacy or find a location where they can walk-in to
get vaccinated. Appointments are available based on
vaccine supply.
To schedule an appointment or find a walk-in location please visit, myturn.ca.gov.
Those who don't have access to internet may call the CA COVID-19 Hotline for
assistance at 1-833-422-4255 (M-F: 8am-8pm, Sat-Sun: 8am-5pm).
Digital COVID-19
Vaccination Card
Individuals who got vaccinated in
California can receive a digital
version of their vaccination card
through the Digital COVID-19
covid-19-test image
Vaccine Record (DCVR) portal.
After completing the form
provided on the portal,
individuals will be sent a link via
email or phone. The link will lead
to a QR code and a digital copy
of their COVID-19 vaccination
record that can be saved or
downloaded.

COVID-19 Home Test Self-
Reporting
Residents of Butte County can self-report
positive results of a COVID-19 test taken at
home using forms developed by Butte County
Public Health. Reporting the positive home
test results will provide more information
regarding transmission rates in Butte County,
which will aid in the detection of possible
surges in cases in the community. Self-
reporting is completely voluntary.

COVID-19 Dashboards
Butte and Glenn County both have their own COVID-19 Dashboard that provides
information about the COVID-19 cases in their own counties. Residents of each
county may view their county's dashboard to stay informed.
Butte County
The Butte County COVID-19 Dashboard provides information about the
COVID-19 cases in Butte County. Information provided on the dashboard
includes:
· the cumulative number of cases in the county
· the daily number of people hospitalized and in the ICU in Butte County
· the proportion of different variants found in Butte County among all
residents who have been tested for variants
· the number of cases by age group among all cases on a weekly basis
· cases by age among those under the age of 18
· vaccination rates in Butte County

Glenn County
The Glenn County COVID-19 Dashboard provides information about the
COVID-19 cases in Glenn County. Information provided on the dashboard
includes:
· the cumulative number of cases in the county
· the number active cases that are in self isolation or hospitalized
· case count per month
· vaccination rates in Glenn County
The dashboard is available on the Glenn County Health & Human Services
