To whom it may concern,
Zayo Group care of Tilson Technologies Management, Inc, is submitting for approval of an
Administrative Permit Application to construct, operate, and maintain a telecommunications facility.
This telecommunication facility will be approximately 200' x 100' fenced compound for new ground
mounted antennas and electrical service fully enclosed within an 8 ft. fence perimeter. Please see
construction drawings associated with the submittal for the comprehensive scope of work.
Please accept the following items as our official submittal:
• Project Description (this page);
• Project Information Form;
• Agent Authorization;
• Receipt of Grant Deed Request from County of Butte Clerk-Recorder. The Grant Deed will be
sent upon receipt; and
• Construction Drawings.
Please provide the invoice for this submittal when ready as we will pay online.
