SPECIAL NOTES:
Inspection of a 1000-gallon concrete tank that is in poor condition, the tank is leaking. The tank was
12” deep. There was 0” of scum and 7” of sludge in the inlet side of the tank. The Effluent level in
the tank is 20” lower than normal operating level. A Hydraulic Load test cannot be done until repairs
are completed. The D-Box is crushed/cracked. There is a line plugged with roots and two lines that
are crushed. See diagram for details.

*** This hydraulic load test of the absorption field
does not constitute a full hydraulic load test of the
dispersal field. A full hydraulic load test can only
be effectively done with more water and when the
system has been in continual use.
