Hi Matt:
I was able to locate the inspection reports that did for each of the sites. Please note at the bottom of each report
you marked a date for the re -inspections to be done. If you have not done the re -inspections please do so
ASAP. If you let me know when you plan to do them I will let the sites know to expect you. If you have already
done them please provide copies of the re -inspections. Thank you for your help with this.

Sherry,
I can not find the forms for either facility. They were both instructed to call and schedule a follow up inspection
and neither did. I do not have sufficient personnel to perform inspections if a facility is not ready to clear
deficiencies, that is why I put it on them to call when they have fixed whatever needs fixing.
Please have them call to schedule another inspection and I will gladly go out and do it.

I do not have the files in front of me for either facility you reference below. However, my procedure for these
inspections is to leave a copy of the inspection form with the facility representative and then they forward to the
appropriate contact within your organization. The follow up inspections are not scheduled at the time of the first
inspection, but merely a date is set approximately a month out and corrections should be done by then. I do not
go out for a re -inspection until I am contacted by the facility to do so.
