This letter presents Pacific Gas and Electric Company’s (PG&E) Part 12D inspection plan,
including an independent consultant (IC) team proposal, for the comprehensive
assessment (CA) of the following dam and appurtenant structures associated with PG&E’s
DeSabla-Centerville Hydroelectric Project, Federal Energy Regulatory Commission
(FERC) No. 803. FERC provided PG&E the general requirements of the Part 12D CA for
the following dam in a letter dated January 22, 2025.

The enclosed Philbrook Dams Part 12D inspection plan (Enclosures 1) includes
descriptions of project features to be inspected, a schedule for inspection-related activities,
and IC team resumes. In accordance with Title 18 of the Code of Federal Regulations,
Section 12.34, PG&E requests approval of the IC teams and potential failure mode and
risk facilitator.
PG&E’s schedule included in Enclosure 1 meets the requirement to have the IC team’s
first field activity occur at least 180 days after submitting the Part 12D inspection plan. With
FERC approval, PG&E and the IC team are available to perform the first project activity on
approximately April 20, 2026, which would include submitting the pre-inspection
preparation report 30 days before the activity begins.
