This letter presents Pacific Gas and Electric Company’s (PG&E) Part 12D inspection
plan, including an independent consultant (IC) team proposal, for the comprehensive
assessment (CA) of the following dam and appurtenant structures associated with
PG&E’s Bucks Creek Hydroelectric Project, Federal Energy Regulatory Commission
(FERC) No. 619. FERC provided PG&E the general requirements for the 12th Part 12D
CA for the Grizzly Forebay development in a letter dated May 15, 2024.

Grizzly Forebay Dam 12th November 1, 2025
The enclosed Part 12D inspection plan (Enclosure 1) includes descriptions of project
features to be inspected, a schedule for inspection-related activities, and resumes for
the IC team. In accordance with Title 18 of the Code of Federal Regulations, Section
12.34, PG&E requests approval of the IC team and potential failure mode and risk
analysis facilitator.
PG&E’s schedule, included in Enclosure 1, meets the requirement to have the first IC
team activity occur at least 180 days after submitting the Part 12D inspection plan. With
FERC approval, PG&E and the IC team are available to perform the first activity on April
22, 2025, which would include submittal of the pre-inspection preparation report 30 days
before this IC team activity.
