A Memorandum of Understanding between the U.S. Department of Interior, BLM; U.S. Department of Agriculture, USFS; the State of California, Department of Conservation; and the State Mining and Geology Board (SMGB), discusses implementation of SMARA on Federal lands in California that are under the jurisdiction of the BLM and/or the USFS. As required by PRC § 2774(b) and CCR § 3504.5(g), Lead Agencies shall file an Inspection Notice that includes a statement regarding compliance with SMARA, a copy of this Surface Mining Inspection Report (MRRC-1) and any other supporting documentation with the Department within 30 days of completion of the inspection. The Lead Agency shall also forward a copy of the Inspection Notice, MRRC-1, and any supporting documentation to the operator. In order to determine what adjustments, if any, are appropriate to the Financial Assurance Mechanism amount, each mine operator must submit annually a revision of the written Financial Assurance Cost Estimate to the Lead Agency (PRC § 3804(c)).Provide the date of the operator’s most recent revision of the Financial Assurance Cost Estimate to the Lead Agency and where appropriate, provide a status of the pending Financial Assurance Cost Estimate. Provide the date and amount of the most recently approved Financial Assurance Cost Estimate. Operations are performed in a manner that would not preclude the site from reclamation in accordance with the approved reclamation plan. a) 1,439 acres b) 20 acres c) 20 acres d) N/A Operations are conducted within the permitted boundary. Upon completion of mining, disturbed areas will be graded to allow for proper drainage, and backfilled with fines recovered from mining activities. The exposed basalt cap will be topped with 3 inches of 'natural materials' for revegetation. Working and reclaimed slopes will generally have 2:1 slopes or flatter, except for the quarry area walls, which will have 1:1 slopes. Two to three benches will be established along quarry walls. Each bench will be approximately 25' wide and 50' high. Mineral extraction are progressing in accordance with the approved reclamation plan and plan of operations. Quarry walls appear stable. Fines from processing activities are being redistributed onto the quarry floors to establish a smooth surface; reducing wear on equipment. In July 2013, a basalt column along the haul road (outside the quarry area) had collapse. The column has not been removed. Its anticipated that the column will be left in place. Erosion will be controlled during all phases of mining activities. Specificperformance criteria and measures are detailed in the Mine's SWPPP.Primary erosion concern is focused on the underlying Ione Formation, which can undermine the basalt formations causing instability. Erosion channels greater than 6 inches wide or 4 inches deep shall have the runoffimmediately diverted to a new, stable location. No erosion was observed. Areas shown to have erosion the previously year appear to have been regraded. Due to the porous conditions of the surface, minimal erosion at the site is anticipated. The mine uses on-site settlement ponds for the process water,which is supplemented by fresh water collected in Chaffin Reservoir and on-site wells at 70,000 gpd. Stormwater runoff within the mine is channeled and collected on-site. Informationfor the existing ponds are contained in the SWPPP. Existing ponds are maintained and in good working condition. The existing freshwater pond is only used for make-up water and stormwater runoff containment due to the of the water clarifier and large storage tanks N/A N/A INSTRUCTIONS FOR COMPLETING SURFACE MINING INSPECTION REPORT Form MRRC-1 (4/97) Page 3 and 4 (Rev. 07/13) BLOCK VIII: INSTRUCTIONS FOR EACH DATA COLUMN: Potential Reclamation Plan Requirements (Column 1): Under CCR § 3504.5(f), “Inspections may include, but shall not be limited to the following: the operation’s horizontal and vertical dimensions, volumes of materials stored on the site; slope angles of stock piles, waste piles and quarry walls; potential geological hazards; equipment and other facilities; samples of materials; photographic or other electronic images of the operation; any measurements or observations deemed necessary by the inspector or the lead agency to ensure the operation is in compliance with Public Resources Code Chapter 9.” Column 1 provides a list of items that may be included in the approved reclamation plan, either expressly or by reference as described in PRC § 2772(d), which may include conditions of approval, other permit requirements and supplementary documents, including environmental documents, prepared for the project pursuant to Division 13 (commencing with Section 21000). It is not expected that all reclamation plans will include each item of Section VIII, or be limited to the items listed. Items in Column 1 that are not operative requirements in the reclamation plan may not need to be addressed by the inspection. Operative reclamation plan requirements not listed in Items 1 through 12 may be listed in Item 13, under “Other Reclamation Plan Requirements.” Reclamation Plan Requirements (Column 2): Prior to field inspection, it is recommended that the inspector review the approved reclamation plan and any amendments, as well as any other documents included by reference, including conditions of approval, other permit requirements and supplementary documents, such as environmental documents prepared for the project pursuant to Division 13 (commencing with Section 21000) that specifically relate to reclamation of the mine site. The most recently approved Financial Assurance Cost Estimate and any pending or ongoing enforcement actions should also be reviewed. Conditions of approval that relate to facility operations solely of local concern, such as hours of operation, noise, and dust control are not subject to the inspection. Column 2 is intended to provide the inspector a place to match any items noted in Column 1 with those items included in the approved reclamation plan either expressly or by reference as described in PRC § 2772(d), which may include conditions of approval, other permit requirements and supplementary documents, including environmental documents prepared for the project pursuant to Division 13 (commencing with § 21000). Also note any Interim Management Plan (IMP) requirements where the mine is subject to an IMP pursuant to PRC § 2770(h). Indicate the source document for the reclamation plan requirements at the end of the entry in parenthesis; i.e. (COA) (POO) (EIR) (WDR) (SWPPP), etc. Conditions of approval that relate to facility operations solely of local concern, such as hours of operation, noise, and dust control should not be included in Column 2. If items listed in Column 1 of Section VIII of the form are not included in the reclamation plan or other documents included by reference, write not applicable or “NA” in Column 2. Specific reclamation requirements may not apply to an operation at the time of inspection, but they are important to be aware of to ensure current activity at the site will not prohibit reclamation in accordance with the approved reclamation plan. A copy of the Surface Mining and Reclamation Act of 1975 and 1993 SMGB regulations may be obtained at http://www.conservation.ca.gov/omr/lawsandregulations/Pages/SMARA.aspx. Site Conditions and Compliance Issues (Column 3): Describe current site conditions and compliance issues noted for both operating and reclaimed surfaces that pertain to the reclaimed condition of the mining site. Evaluations of slope stability and engineered compaction should be prepared by qualified professionals only. Any available topsoil and subsoil will be saved and stockpiled for reclamation uses. All topsoil will be stripped in the initial phase. Topsoil and subsoil will be stockpiled separately. Stockpiles will be temporarily revegetated and will not be disturbed until redistribution. Any overburden not used for commercial sale will be stockpiled and used for reclamation. Approximately 817,000 tons of overburden is anticipated to be produced. All drainage will be diverted away from the stockpiles, and stockpiles will be inspected periodically during operations for stability and erosion control. There will be no permanent waste dumps remaining at the conclusion of mining activities. Once mining is complete, soils will be spread evenly (minimum 3-inch depth) over disturbed areas. Available top soil located on the top plateau (Table Mountain), above the quarry area are scraped aside with a bull-dozer, prior to mining activities. Top soil stockpiles are kept separate from material and overburden stockpiles. Stockpiles have naturally revegetated. No erosion was observed. A Hydromulch Dryland Mixture will be applied on flat areas at the rate of 100 lbs seed: 35% Blue Wild Rye, 25% California Brome, 10% pygmy lupine, 5% Clover, 5% CaliforniaPoppy, 10% Deer Grass, 10% Molate Red Fescue; 225 lbs 15-15-15 Fertilizer; 80 lbsTackifier; and, sufficient water to form slurry. An augented hydromulch seed mixture willbe applied onto headwall slopes. The augmented mixture is consistent with the previousmixture, but is augmented with 5 lbs containing at least 4 of the following species: CoyoteBrush, Redbud, Toyon, Coffeeberry, Deer Brush, and Flannel Bush, and 1 lb. Live Oak.Hydroseeding shall occur in the fall to coincide with the first significant rain event. Jutenetting will be installed on all slopes exceeding 3:1.Revegetation efforts will be monitored until reclamation is complete and accepted. Reclamation will be complete when the site contains less than 10% weeds and has aminimum of 300 lbs residual dry matter. A certified, weed free, native seed mixture will be used on reclamation areas. Annualevaluation of weed growth on reclaimed areas will be performed. If weeds comprise ofmore than 10% of vegetative growth during the first two years after revegetation,appropriate control measures will be implemented. Control of weeds may include activecontrol of established weeds; control through land management; minimizing newdisturbance of soil and vegetation; or, timely restoration of disturbed areas. Revegetation of mined slopes in Areas 5, 6, 7 and 8 were performed between 2004 and 2008. Slopes are revegetated however, noxious weeds are present on the site, but below 10% thresholds. Additional weed abatement may be necessary to achieve satisfactory reclamation results. Previous oak tree plantings were also performed in accordance with use permit requirements. According to No information is available on the success of these plantings. All equipment, temporary structures, and permanent structures will bedismantled and removed from the site at the conclusion of mining activities. The lead agency may cause an inspection to be performed by contracting with private consultants, specifically: state licensed geologist, state licensed civil engineer, state licensed landscape architect, or state licensed forester per § 2774(b). 2015 Annual Inspection Report  Table Mountain Quarry (91‐04‐0011)  1    OBSERVATIONS   The mine was active during inspection with mineral exporting and blasting.  No mineral  processing was occurring during inspection.  Mine operations were performed throughout the  year, with blasting occurring approximately once a month.  Excavations continue to occur along  the top tier of the quarry area.  Excavations conform to the approved plans.   Blasting of rock is limited to between the hours of 10 am to 3 pm, per Condition #8 of mining  permit (MIN) 04‐01.  Blasting occurred at approximately 12:15 pm, which is consistent with the  approved mining permit.  Blasting was not in the direct line of sight to any residential dwellings  (Condition #9).   An annual compliance report covering all conditions and mitigation measures is required to be  prepared by a qualified consultant, hired by the operator (Condition #16).  An annual  compliance report for the quarry was prepared by Sespe Consulting, Inc. on November 14, 2014.   The report concluded that operations are consistent with the approved plan of operations and  permit conditions of approval.  No report has been submitted for 2015.        Drainage swale graded along the upper portion of the on‐site haul road between the excavation  area and processing area.  The drainage swale was created on the cut side of the haul road, and  terminates midway down the road where drains into a containment area with raised berms.   Eight total new blue oak saplings were planted on a berm, located between the railroad and the  mine office, to account for the loss of blue oak saplings reported in 2014.  Seven of the oaks  were planted on the south side of the berm, and one oak was planted on the north side of the  berm.  The plantings occurred in late spring of 2015.    A Mitigation Monitoring Report was prepared by Foothill Associates to review the tree  revegetation efforts performed on the mine site.  The report indicated that the site was  surveyed on October 27, 2015 by an International Society of Arboriculture (ISA) Certified  Arborist.  The survey verified that 12 blue oaks (includes the 8 oaks planted) and 1 redbud were  observed along a berm between the railroad and the mine office.  The presence of the 12 blue  oaks meets the minimum 9 blue oaks required to be replanted at the site.     The Mitigation Monitoring Report recommended that tree tubes be installed to protect the  saplings from animal grazing, and to water on a regular basis during hot temperatures.     All on‐site hazardous materials are stored in the designated Fuel Storage & Containment Area.   No obvious issues were observed with the storage area or stored materials.  VIOLATIONS/CORRECTIVE MEASURES  No violations or corrective measures to report.   September 16, 2015 Annual Inspection – Table Mountain Quarry (91-04-0011) Photo 1 – View of the top tier of the excavation area.
